Position Summary
Phoenix Children’s has a unique opportunity for a highly specialized unit that requires the ultimate skill set in dealing with families/patients during the most challenging time of their lives. This Nurse Care Coordinator position is a full time position, M - F. The incumbent will be working with the Nurse Practitioner, a social worker and a team of physicians who all specialize in Palliative Care. Incumbent will also collaborate with all Specialties across the organization to provide the best care.
The ideal candidate will have a minimum of 2 - 5 years of experience in the in/outpatient care; working with patients who are medically complex; Palliative Care experience is a plus. This is a unique opportunity to work with a highly skilled team that works closely together to treat the most vulnerable children seeking care at Phoenix Children’s Hospital.This position utilizes advanced critical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care. It provides leadership, clinical expertise /integration and functional direction to team members in order to ensure quality and continuity of patient services. Serves as the expert resource within a specialty area and participates in assessing specialty population needs and health outcomes. It facilitates continuity of care using nursing process, interprofessional collaboration, coordination of and access to appropriate health care services and community resources.
Education
- Four year degree (BSN) in nursing, or a Diploma Nursing degree or two year (ADN) degree in nursing coupled with directly related work experience. (Required)
- Masters degree in Nursing. (Preferred)
Experience
- Three years of clinical practice experience. (Required)
- Experience with pediatric or specialty populations. (Required)
Certifications
- Current State of Arizona RN License from a compact state with multistate privileges or the ability to establish licensure according to Arizona law prior to start date. (Required)
- Specialty and/ or Care Coordination and Transition Management Certification. (Preferred)
- Current BLS Certification for Healthcare Provider from the America Heart Association. (Required)
