Non-Medical Care Coordinator
Job Summary: The Non-Medical Care Coordinator is responsible for identifying patients living with HIV in need of care, and working with these patients to overcome Social Determinants of Health (SDOH) barriers interfering with their ability to regularly engage in care. Responsibilities: score: N/A Assists individuals not engaged in medical care in entering into the healthcare system by acting as a patient advocate and navigator. Coordinates with external organizations to connect enrolled clients with services needed to address the SDOH.
