The National Association of Community Health Centers (NACHC) is the national membership organization for the nation’s federally qualified health centers (also known as FQHCs or Community Health Centers). POSITION DESCRIPTION
The Director, Value-Based Care will direct, oversee, and strategically coordinate national level initiatives focused on value-based care programs, trainings, reporting, and deliverables. The individual will work as part of a team to lead a portfolio of value-based care programs and activities to support health centers advancement toward the Quintuple Aim goals of improved health outcomes, improved patient experiences, improved staff experiences, reduced costs, and equity.
RESPONSIBILITIES
- Direct the day-to-day operations of NACHC’s value-based care (VBC) and transformation programs and activities.
- Direct development of education, reporting, tools, and transformation related work plans supporting health centers transition to value-based care.
- Oversee creation of best practice workflows and process mapping tools to support health center value-based care trainings and resources.
- Lead development of financial risk models for health center analysis of payer proposals, shared savings/shared risk, capitation, and percent of premium contracts.
- Develop tools to support health center assessment of risk exposure, and potential upside and downside opportunities.
- Develop templates for health center management reporting systems and dashboards to measure, track, and monitor performance within value-based contracts.
- Oversee NACHC’s internal value-based care (VBC) cross-divisional workgroup and alignment around VBC training, programs, and deliverables.
- Oversee NACHC’s VBC educational content and work cross-functionally to maximize impact across divisions and meet educational needs by audience/segment.
- Contribute to development of innovative health center care models and value-based care programming and contracting.
- Develop VBC education and resources for NACHC staff and Health Center Program partners.
- Prepare reports and presentations on value-based care and transformation research, policy, evidence, and trends.
- Coordinate activities using the Value Transformation Framework and its efforts to support health centers in practice transformation and participation in value-based care and other sustainable business models.
- Support partnerships with primary care associations (PCAs) and health center controlled networks (HCCNs) and alignment in support of systems transformation and population health.
- Lead coordination and collaboration of NACHC divisions on VBC priorities and initiatives.
- Represent NACHC with other organizations and agencies around VBC projects and priorities.
- Lead meetings with key external partners, including Centers for Medicare and Medicaid Services’ (CMS) Center for Medicare and Medicaid Innovation (CMMI).
- Oversee adherence to funder expectations on deliverables, reports, and budgets.
- Oversee grant writing, reports, collecting and documenting deliverables, and monitoring invoices.
- Supervise staff, as assigned.
- Ensure products are delivered on time and of high quality.
- Other duties as assigned.
REQUIRED QUALIFICATIONS:
Education
- Master’s or advanced degree preferred.
Experience/Ability/Knowledge
- At least 5 years’ work experience in value-based models and managing implementation activities and performance outcomes.
- Knowledge of value-based program strategies, cost of care initiatives, and performance analytics.
- Experience with VBC contract negotiations and contract management.
- Ability to read and interpret managed care value-based contracts and experience in managing value-based reimbursement initiatives.
- Demonstrated project management skills; project management certification a plus.
- Experience with NCQA HEDIS, CAHPS Measures, and CMS Value-Based Care programs desired.
- Excellent Microsoft Office skills.
- Advanced analytic skills.
- Experience with software to support project management, such as Asana or other tools.
- Experience in development and use of learning management systems a plus.
- Experience in organizing and displaying data using such programs as Salesforce, Tableau, and Microsoft Power BI.
- High degree of independence, flexibility, initiative and commitment.
- Proactive, motivated, comfortable working in a fast-paced environment, self-manages multiple priorities simultaneously under deadlines.
- Accuracy with work, strong organizational skills, and attention to detail.
- Experience with health centers, a plus.
- Ability to self-manage multiple priorities, simultaneously, under tight deadlines.
- Excellent written/verbal communication skills.

- Willing and able to travel approximately 10% of the time.
