This strategic and visionary position is accountable for preparing Empath Health Care Navigation to be competitive in, and a driver of, the future of healthcare. This patient-centric role must excel at navigating the managed care ecosystem and forecasting changes in payer mixes and expectations, quality benchmarks and the financial impacts associated with them, as well as the key drivers in creating winning partnerships with payors. In addition to providing the strategic direction and operational oversight of Care Navigation, Call Center and Admissions Departments and Programs, this key role drives and owns organizational growth, in partnership with key stakeholders throughout Empath Health. A key deliverable of this role is ensuring efficient and effective processes and systems throughout Care Navigation are optimized to deliver full life care service excellence as patients, clients, participants and their families entrust us with their care and navigate through our care continuum. The successful candidate must exude our Mission while building and maintaining effective relationships with referral sources and community stakeholders to Empath maintains a strong, positive community presence.
Primary tasks, duties, and responsibilities:
1. Sets the vision for Care Navigation and the patient experience for traversing our continuum of care. Enables partnerships internally and externally to facilitate that vision. Establishes, measures and takes proactive action on key Care Navigation metrics. 2. Establishes this role as one of the key strategists for providing the Agency expert prognostication, guidance and advice to appropriate address changes in managed care initiatives, value-based programming, payor mixes and regulatory-driven programming. 3. Drives exceptional quality and compliance outcomes in all Care Navigation processes and systems, ensuring the highest of standards are met. 4. Continuously monitors and makes appropriate adjustments to departmental processes to ensure flawless execution of Care Navigation across our continuum in collaboration with leadership from across the Agency. 5. Ensures all aspects of Care Navigation are meeting or exceeding growth, quality and community relationship goals of Empath Health in relation to the Certificate of Need (CON), Agency for Health Care Administration (AHCA), Joint Commission (JC), Office of Inspector General (OIG) and community partners. 6. Monitors the current state of healthcare system to identify opportunities and risks to vision and mission of Empath. Communicates to seek regular input from the organization’s key constituencies and community partners regarding the quality of programs and services and the organization’s relevance and opportunity to be innovative and improve process and services. 7. Ensures the strategic goals of Empath are carried out by the departments, specifically through leadership, vision and execution of the key initiatives related to organizational growth. 8. Promotes and practices the Empath vision, mission and core values, and follows all policies and procedures of Empath organization. 9. Mentors and promotes the professional development of direct reports and monitors professional development and performance of all colleagues in department. Implements changes as needed to ensure individual and department success. 10. Collaborates with leadership, direct reports and peers to respond promptly to resolve internal and external issues. Follows up with appropriate staff/departments to solve issues. Communicates with internal and external stakeholders to reach consensus and execute activities that foster solid relationships. 11. Develops and manages department budget, demonstrating fiscal responsibility.

• Bachelor’s degree in Business, Marketing, Nursing or related field required; Master’s Degree in profession or healthcare administration or business preferred. • Minimum five years of progressive healthcare leadership experience preferred, with three years of program planning in access and admission and/or business development experience. • Three to five years expertise in managed care environment to understand, evaluate and craft appropriate actions plans for shifts in reimbursement models. • Proven experience of successfully achieving incremental revenue and profit growth in a for-profit organization. • An equivalent combination of professional experience and education may be considered as meeting the Education/Professional requirements. • Excellent strategic thinking, business development and innovation skills. • Exceptional focus on delivering top box customer experiences and continually improving processes to excel in the future. • Deep desire to coach, mentor, motivate and train colleagues to provide greatest opportunity for achieving full potential. • Characterized by leading by example, displaying a calm approach in all situations, reacting appropriately to a dynamic and fast-paced environment and being an empathetic listener. • Keen influencing skills, especially for situations where there is no reporting relationship. • Nimble decision-maker, quickly assimilating large and disparate data points into executable strategies. • Possess a valid Florida driver’s license and current auto insurance.
