Position Summary
The goal of care coordination is to assist in managing care, cost, and outcomes across the continuum of care. The Manager of Population Health supports the Clinically Integrated Networks Disease Management, Utilization Management, Case Management, and Population Health initiatives. This position works collaboratively with the Director of Clinical Integration in developing and implementing an electronic health information system. Additionally, this position works with data analysts to provide the care coordination team high level technical and analytic support. This includes, but is not limited to, identifying high cost/high risk members and performance analysis, which will guide the care coordination team.
Education
- Bachelors degree in Nursing. (Required)
- Masters degree in healthcare administration, business management, or related field. (Preferred)
Experience
- Minimum of 2 years of broad clinical experience. (Required)
- Experience with EMR implementation. (Required)
- Experience with healthcare data analysis. (Required)
- Experience with care management delivery programs. (Preferred)
Certifications
- Current State of Arizona RN license from a compact state with multistate privileges or the ability to establish licensure according to Arizona law prior to start date. (Required)
Special Skills
- Excellent verbal, written and interpersonal communication skills. (Required)
- Effective critical thinking and decision-making/problem solving skills. Ability to prioritize and handle multiple tasks in a demanding work environment. (Required)
- Intermediate to advanced skills in Excel and PowerPoint. Competent in data manipulation and analysis. (Required)
- Strong knowledge of reporting, data analysis and information presentation. (Required)
- Experience implementing and supporting electronic health records. (Required)
- Ability to work independently and on a team. (Required)
