The Stroke Coordinator is responsible for the coordinating of quality, cost effective patient care by coordinating the interdisciplinary team to respond to variances in care and to provide resources through the team to patients and families.

DUTIES AND RESPONSIBILITIES:
Abstracts all patient records admitted to the hospital and the emergency room for pre determined quality / performance metrics. Metrics are defined by CMS, Get with the Guidelines, and the Stroke Council.
Enters data into Midas and Get With the Guidelines within 10 days of discharge.
Generates quality data reports for the stroke council and the quality committees within the organization.
Provides input on variances to the data to the above teams.
Takes immediate action on variances as noted in the performance and quality metrics by contacting physicians and management as indicated.
Is responsible for scheduling of stroke council, working closely with the Medical Director;
Reserve room, set agenda and distribute, assure invitations are sent to all members, arrange for lunch, encourage attendance and participation of all members, keep and distribute minutes. Provide for supporting documents at the meetings.
Provides education to all level of care providers on the stroke care pathway and essential components of stroke care
Coordinates and implements process improvement for stroke care, including developing annual plan
Coordinates efforts in meeting regulatory compliance, federal, state and local regulations and standards
Communicates appropriately and clearly to physicians, staff and administration.
Provides input to leadership on budgetary, staffing and other operational needs.
Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to handle constantly changing situations.
Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict.
Professional Requirements:
Adheres to dress code.
Completes annual educational requirements.
Maintains regulatory requirements.
Wears identification while on duty.
Maintains confidentiality at all times.
Attends department staff meetings as required within the department.
Reports to work on time and as scheduled; completes work in designated time.
Represents the organization in a positive and professional manner.
Actively participates in performance improvement and continuous quality improvement (CQI) activities.
Coordinates efforts in meeting regulatory compliance, federal, state and local regulations and standards
Communicates and complies with the Benefis Health System Mission, Vision and Values as well as the focus statement of the department.
Complies with Benefis Health System Organization Policies and Procedures.
Complies with Health and Safety Standards and Guidelines.
High School Graduate or equivalent.
Experience with data analysis, research and outcomes measurement preferred.
Experience in Stroke care preferred.
