Position Summary:
The Ambulatory Care Manager is accountable for the development of care plans intended to manage selected high-risk patients. This work is focused on closing care gaps and aiding these patients in navigating the complex health care landscape. Working with inpatient and ambulatory providers, physicians, peers, facility and community colleagues, this role identifies realistic and actionable patient centered care goals and facilitates their demonstrable achievement through the development and implementation of a written, patient-specific care plan. This position requires extensive internal and external contacts with patients, staff, providers, management and community resources.
The Ambulatory Care Manager is an experienced clinical nurse who has the ability to interpret information from a variety of sources to delineate a focused patient-centered care plan. As well, the Ambulatory Care Manager is accountable for the achievement of this care plan resulting in graduation from care management generally within a 90 (ninety) day period. This role also requires substantial clinical competency & specialized knowledge, judgement and skill, based upon the principles of comprehensive patient care. This position requires self-direction, motivation, organization and interpretation of information.Minimum Requirements:
Minimum of three years experience in ambulatory care setting required
Knowledge of care continuum (inpatient, ambulatory, post-acute, etc.) preferred
Consideration may be given to internal candidates not meeting the minimum qualificationsEducation/Licenses/Certifications:
Current registration as a professional nurse in the state of Washington
BSN required or acquired within 3 years of hire into the position
