The community you’ll be caring for: AdventHealth Carrollwood
The role you’ll contribute: The Readmission Coordinator, in collaboration with the CMO, CNO and Director of Administrative Improvement, will implement AdventHealth Carrollwood’s Readmission Reduction Program. This program will target patients who are readmitted to the hospital within 30 days of discharge and fall into a priority diagnostic group as identified by CMS. Patients will be identified by various methods including collaborating with the Transition of Care Team members and Medical Providers. Transitional management will be provided to the patients who have the highest risk for readmission and strategies will be appropriate to that level.
The value you’ll bring to the team:
Responsible for the development, implementation, and maintenance of Readmission Reduction program.
Collaborates to reduce readmission rates as it relates to the CMS targeted diagnosis.
Communicates with all providers and care team if high risk readmitted patient arrives at the hospital (ED or other) in an effort to avoid the preventable readmission (i.e., evaluate until admitting practitioner sees patient, provide outpatient options, etc.).
The expertise and experiences you’ll need to succeed:
Bachelor’s Degree in Registered Nursing
Minimum of five years of experience in a healthcare setting, of which a minimum of two years has been spent in an acute care setting
Valid FL RN License
Master’s Degree in Advanced Practice Registered Nurse (APRN)
In-depth knowledge of utilization management criteria, complex medical and surgical patients, reimbursement, and regulatory requirements including CMS, TJC and ACC guidelines.
Working knowledge of current care practices for the diagnostic groups identified by CMS.
