SOS is proud to be the largest Federally Qualified Health Center in Orange County to combine wrap-around social services with health care. We have 6 community health centers in Costa Mesa, Newport, Anaheim, Santa Ana and Mission Viejo.
The Quality Improvement Project Manager is responsible for the coordination and implementation of the Continuous Quality Improvement (CQI) Program along with other innovation initiatives/programs for the organization. This will include the collection, management, performance improvement and analysis of quality data and the preparation of quality reports and recommendations for improvement. Utilizes knowledge of LEAN tools of improvement and techniques of Continuous Quality Improvement, including analysis and interpretation of data using computer based disease registries or similar data collection systems is essential. The Quality Improvement Project Manager will participate in and assist in preparation for clinical audits and surveys. Leads and/or participates in a variety of improvement and innovation projects. The Quality Improvement Project Manager works in the team-based model of care.
Contributes to the collaboration of health professionals and members of the expanded health care teams in order to support the patient to attain health goals.
Review and analyze clinical quality measures, patient and staff experience scores and other data for improvement.
Champion the patient experience charter; liaison for the patient experience committees
Evaluates processes for performance improvement across the enterprise and collaborates with other departments for continuous quality improvement and innovation.
Collaborates with all levels of management to help improve and streamline process for patients.
Provides in service training to providers and staff about quality and the tools for continuous quality improvement.
Prepares QI reports, statistics and updates for projects at meetings such quality improvement committee meetings and board CQI
Project manage specific improvement and innovation grants by tracking, developing, and maintaining internal reporting systems, maintaining excellent historical records, optimize grant work-flow and provide the data required for reporting.
Participates in staff and management meetings as they relate to QI and improvement activities
Reviews QI tools and surveys and provides technical assistance to staff
Coordinates and facilitates CQI meetings.
Assists in the development, monitoring, and presentation of internal quality measures and initiatives
Coordinating tracking and reporting of clinical outcomes, and follow-up of corrective action plans
Assists in developing methods for data collection and extracts data as required
Evaluates programs/initiatives for effectiveness and ensure compliance with state and federal regulatory agencies.
Coordinate corrective action reports when appropriate.
Work with Managed Care Plans in mitigating clinical quality gaps.
Represents SOS with co-venture partners. Attends meeting, tracks developments, and deliverables required.
Assist with annual UDS, HRSA and other reports as determined.
Supports, administrative and development of PCMH and other improvement initiatives submission/renewal.
Produces summary program reports summarizing key goals, accomplishments and outcomes on monthly intervals.
Assists with audits for regulatory requirements (OSV, FTCA, UDS, OSHPD, etc)
Facilitates patient and staff experience and initiatives
Coordinates a team approach to review utilization of resources.
Stays current of new developments and industry trends related to PCMH, clinical quality improvement and other improvement programs or initiatives.
Attends conferences and training sessions as requested
Attends staff and clinic meetings as requested.

EDUCATION AND/OR EXPERIENCE:
Bachelor degree required
Masters in Public Health, Healthcare or Nursing preferred but individuals with experience leading a Quality Improvement program may be considered
LVN or RN License, current to practice in California is strongly preferred
Certified Professional of Health Quality (CPHQ) preferred
Minimum 2-3 years of progressive experience in quality improvement and/or utilization management in a healthcare facility, preferably with a federally qualified health center doing business in California.
Experience in health plan developing and performing case management/disease management
Experience working with the Managed Care Medi-Cal and Medicare populations
Experience working with disadvantaged populations helpful and knowledge of health disparities preferred.
Experience directly managing grant application and/or processes
Must be familiar with PCMH accreditation and processes
Must demonstrate integrity, sound judgment, demonstrated leadership skills, and strong interpersonal skills.
Must be able to approach staff about quality issues with tact and diplomacy.
Proficiency with Microsoft Office Suite (Excel, Word, Powerpoint, Access, etc),
Excellent skills in data collection analysis, and presentation to include the creation of advanced level spreadsheets for QI-related data management and display
Proficient use of Practice Management systems & EHR Softwares
Excellent organizational skills and attention to detail.
Ability to establish work priorities to meet established deadlines
Demonstrated ability to develop and maintain positive relationships with clinic and transformation support staff that promotes cooperation.
Excellent public speaking and presentation skills
Bilingual in English and Spanish is preferred
