We are currently seeking a full time Vice President of Population Health Operations to oversee all functions related to Population Health, including direct oversight of quality, data analytics, and clinician engagement. The successful candidate will be responsible for our Population Health Analytics team, Population Health Informatics team, Clinical Engagement team, and our Ambulatory Clinical Documentation team.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Design and implement Population Health strategy to position ChristianaCare and associated network to move purposely and successfully towards total cost of care and enhanced value for the community.
- Lead operations functions related to Population Health, including data analytics, informatics, risk coding, clinician engagement, contract management and network development.
- Develop and enhance partnerships with payers, employers, health systems, and external stakeholders to accelerate success in total cost of care models.
- Work closely with quality structure in the Medical Group and CareVio to align metrics and drive success in chronic disease management, health screening, and care coordination.
- Partner with Population Health leadership group to align clinical and operational initiatives and prioritize interventions for greatest impact.
- Participate in development of payer strategy and contract structure to align with organization's goals.
- Assist with identification and implementation of population health tools and analytic platforms.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Master's/advanced degree in Business Administration or similar degree preferred.
- Minimum of ten (10) years' experience in a healthcare operations leadership role.
- Minimum of five (5) years' experience working in a health plan; experience in a leadership role preferred.
- Experience in population health preferred.
